Workers’ Compensation claims must be authorized by your employer. Before your appointment, please provide the following:
- Workers’ Compensation claim number
- Date of injury
- Names, addresses, and telephone numbers of your employer, your immediate supervisor’s name and the Workers’ Compensation insurance carrier
